We want your first visit to The Village Medical Practice to be a happy, lasting memory. So for that reason, we would like to draw the following to your attention:
All new patients must bring photo ID to their appointment. Unfortunately, appointments cannot proceed without photo identification.
What to bring to your first appointment:
Medicare card and any concession/DVA cards
Photo ID
Previous medical records or specialist letters (if available)
A list of current medications (including supplements)
Please note that The Village is a private billing GP practice, and fees apply to all consultations, and higher fees apply out of hours and weekends/public holidays. Our policy on fees is listed here, and we explain why we need to charge fees here.
Please note, none of our doctors will prescribe Schedule 8, Opiate or Benzodiazepine medications to patients they haven't personally cared for in the past.
All immunisation, surgical procedure and dressing appointments must also be booked with a nurse. To avoid rescheduling please phone the practice on 08 6285 0999 to arrange the correct appointment.
Appointments can be booked via our website (clicking ‘Book Now!’ above), via HotDoc or by calling the practice on 08 6285 0999 during our opening hours (listed below).
Cancellations with less than 1 hours notice, or no-shows will be charged for the full appointment time booked. Further appointments cannot be booked until these fees are paid.
Appointments run for 15-20 minutes depending on your GP, or in increments of these amounts. Fees are charged in these multiples also.
We have parking right outside our front door, with easy access ramps and level entry doorways.
